Achieving Balanced Health 4 (April 2011)

This is the strategy of NHS Sheffield and the GP Practice Based Commissioning Consortia for 2011/12.  It sets out our actions to reduce health inequalities and achieve recurrent financial balance in the NHS in Sheffield.  It is based upon Achieving Balanced Health 3 and keeps the priorities described in that document.

The NHS faces an unprecedented financial challenge, to meet increasing demand for care with no real growth in resources expected in the next few years.

Sheffield uses hospital care more than most parts of the county, with evidence that we have more emergency admissions, more outpatient attendances and more people over 75 in hospital than necessary.

This strategy therefore focuses on actions to reduce our use of hospital services, providing more care in community settings or in people’s own homes, so that hospital care is used only when the specialist expertise and facilities are needed.

It also sets out the actions we will take to improve the health of the population of Sheffield and reduce health inequalities, focussing on:

  • Coronary heart disease
  • Chronic respiratory disease
  • Stroke
  • Diabetes
  • Mental health
  • Smoking
  • Alcohol
  • The health of children and young people
  • Inequalities in life expectancy thought targeted work in priority areas
  • Cancer

Using available evidence on the efficiency and effectiveness of health services in Sheffield, we have established seven programmes designed to achieve significant change and efficiency, set out below.

  • Supporting People to Stay Healthy
  • Reducing Variation in Elective Care
  • Effective use of Urgent Care
  • Long Term Conditions - Supporting People to Stay at Home
  • Continuing Health Care and End of Life Care
  • Effective use of Medicines
  • Mental Health and Learning Disabilities

Annex 1 of this strategy sets out the projects within each of these programmes.

Achieving Balanced Health 3 (February 2010)

Achieving Balanced Health 3 is the NHS Sheffield strategic plan for 2009/10 - 2013/14. It sets out how we intend to achieve our objectives of reducing health inequalities and improving health in Sheffield, and ensuring health services in the city remain affordable so that everyone gets the highest quality and most personalised service possible.

The Big Health Conversation
