Enquiry form

This form can be used to make an enquiry to the Library and be sent in paper format or by e-mail. You can also e-mail us without the form for any small requests.

Literature search request form

This form is for requesting a literature search and can be sent to the library in paper format or by e-mail. Please complete all sections of the form.

Membership form

Use this form if you want to join the Library. This form must be printed out, completed and sent to the Library in paper format as we need your signature.

Photocopy request form & Book loan request form

These forms are for obtaining photocopies of journal articles or book loans from other libraries. The appropriate form should be printed out, completed and sent to the Library in paper format as your signature is needed to comply with the Copyright Act. Non-Sheffield PCT staff will be charged for this service. Please contact the Library for further details.

Feedback/complaints/compliments form

This form is for any comments, complaints or compliments which you may wish to make regarding the services offered by the Library. It can be sent to the Library in paper format or by e-mail.
