The Community Intermediate Care Service (CICS) was introduced in response to a consultation with Sheffield residents that revealed most people would prefer to stay at home and avoid going into hospital if at all possible if they had experienced a period of ill health. Or if they did require hospital treatment they said they would like to leave as soon as they were fit enough, and receive their treatment in their own home.
The service is delivered by Sheffield Teaching Hospitals, Community Services and has been developed to provide better treatment and support for people so they can be cared for in their own home. Some patients may have a short stay in the Beech Hill rehabilitation unit as part of their rehabilitation process.
A multi-professional team including nurses, physiotherapists, occupational therapists, speech and language therapists, clinical psychologists, dieticians, podiatrists, pharmacists, geriatricians and social workers work together to provide the best support for patients.
On referral into the service, all patients undergo a detailed assessment at an early stage so their needs can be determined and the right treatment quickly put in place. This includes checking for more complex health needs such as mental health and learning disabilities which can cause a barrier to treatment if not identified.
Once an assessment has been carried out, a tailored package of care is then developed for each patient.
In addition to treatment, personal support such as making meals can also be provided to help patients manage at home.
Patients also benefit from the allocation of a case manager that coordinates their care and ensures that it is delivered in accordance with their personalised care plan.
This unique, holistic approach ensures patients receive all the care they need at the right time by one service and helps those who can, to recover more quickly.
