Sheffield Practice Based Commissioning (PBC) Consortium Health Profiles 2009/10
Sheffield PBC Consortia Health Profiles 2009/10 present a series of 200 indicators for each Consortium showing relative position to other Consortia, and statistically significant difference to Sheffield values. The profiles are in the form of a ten page document with charts, maps and tables.
The profiles include data on : Population (including distribution map), Ethnicity, Deprivation, Accessibility, Births and Deaths, Quality and Outcomes Framework , Disease Prevalence, Primary Care Activity, Prescribing, Hospital Admissions and Referrals, Vaccination & Immunisation, Screening, Lifestyle e.g. smoking, alcohol.
