The following information is designed to assist you through the process of undertaking your appraisal. You can find additional information and contact details here.
You will be contacted by email 8 weeks before the date on which your appraisal is due. The email will give you details of the timetable you should follow in order to ensure that your appraisal is completed on time.
With approximately 500 GPs to appraise within each financial year, it is essential that these timeframes are adhered to as far as possible. If you anticipate any problems with timely completion, ie maternity leave, sickness, extended leave, sabbatical etc, please inform us by contacting:

You will need to choose your appraiser within 4 days of being informed that your appraisal is pending. Details of all the registered GP appraisers are available here.
If you are a GP Retainer or on the Flexible Careers Scheme you should undertake your appraisal during practice time. You are therefore not eligible to claim the £290 Locum GP fee. Part time salaried GPs who also undertake some locum sessions should undertake their appraisal during salaried hours. Please bear this in mind when setting a suitable date for your appraisal.
Please remember that you should not be appraised by an appraiser that you are in partnership with or by the same appraiser for more than 3 consecutive years (though most GPs would choose to change after 2 years).
When you have chosen your appraiser, please make contact with them directly in order to arrange a date.
You will also need to contact the GP Appraisal Administration Team (see contact details above) to confirm your chosen appraiser and date of appraisal. If you do not confirm your chosen appraiser within the given timescale, the PCT will write with one further reminder but after this will allocate an appraiser to you.

Much of this data is common to practices and can be completed with the assistance of administrative staff. Please attach a copy of the last 4/6 weeks practice rota to give the appraiser an idea of your workload.

This form is to be completed and sent with any supporting documentation to your appraiser at least 2 weeks before the appraisal date. You can begin collecting data straight away. We recommend that you take at least half a day of protected time to finalise the information, complete the form and check this before sending.

These forms look back on the last year’s learning, summarising what you have achieved, what has had to be dropped (if anything) and why. They then lead you to look at how you learn best in preparation for the coming year’s development.

You are advised to keep a copy for your information and also hold all the other forms securely for reference at your next appraisal.
Following appraisal, Form 4 (including summary and PDP) needs to be returned to the PCT within four weeks of the appraisal date. Your appraiser will give you a confidential, pre-addressed envelope in which to submit your Form 4. Please remember to return Form 4, duly completed and signed to the PCT within four weeks of your appraisal.
If you are a locum you are entitled to make a claim to the PCT for undertaking appraisal. This is at a standard rate, which is currently £290. When you have completed your appraisal please submit a duly completed hard copy claim form to the GP Appraisal Administration Team (see details above). Your claim will then be processed. Payment will be made by cheque and posted to the address shown on the completed claim form.
As outlined in Step 2, please remember that GP Retainers, those on the Flexible Careers Scheme and part time salaried GPs who also undertake some locum sessions are expected to undertake their appraisal during practice/salaried time and are therefore not eligible to claim.
