NHS Health Checks are part of a new national scheme which aims to prevent the onset of health problems such as heart disease, stroke, type 2 diabetes and kidney disease.

If you are between the ages of 40 and 74, live in Sheffield, and DO NOT suffer from any of the above conditions, we want your views about how you'd like the Health Check scheme to look in Sheffield.
